or you can send it to school with your child.
SOME TIPS FOR FILLING OUT THE APPLICATION SO IT IS NOT RETURNED TO YOU FOR CORRECTIONS:
1. Remember to include all persons living under the household roof.
2. Do not forget to sign the form.
3. Do not forget to include your social security number.
4. Do not forget to include the food stamp number for each child. NOT YOUR BRIDGE CARD NUMBER!
5. Please fill out your address and phone number completely for faster processing.
We are using a Family Application, this means you only have to fill out ONE application
for all your students.
Remember, you must fill out a new application each year to remain eligible for the free or reduced meals
benefit. To not have a lapse in benefits, you must fill out this form and have it turned in and approved by September
30, 2003.
